Medical evidence

• A University of Salford medical evidence form (DOCX, downloads file), completed and signed by a medical professional 

• Documents from Disabled Students Allowance (DSA) – such as your DSA Needs Assessment, a completed DSA medical evidence form, or your DSA2 letter

• A doctor’s letter

• A diagnostic report (such as for ADHD or autistic spectrum conditions)

• Evidence of a referral for an ADHD or autistic spectrum condition (ASC) assessment. We understand that it can take a while to get an autism or ADHD assessment, so a referral allows you to get support from our service

• A copy of your medical history that shows your name and disability/s (e.g. screenshots from the NHS app)

• A photo of a prescription/prescribed medication which you take for your disability showing your full name and the date prescribed (e.g. SSRIs, anti-seizure medications)

• An in-date sick note from a GP

• A hospital discharge letter
• An occupational health report

• Medical evidence provided by your Wellbeing Adviser or counsellor

• An Educational Psychologist Assessment or report by a specialist teacher
• An Educational Health and Care Plan (EHCP)
• A Form 8

The University can refer eligible students for an Educational Psychologist Assessment for Specific Learning Difficulties (SpLD), such as dyslexia. This assessment can provide a formal diagnosis where appropriate, along with a detailed profile of skills and learning needs.

This assessment is useful for those wanting to explore a possible diagnosis. It’s also necessary when applying for funded support like the Disabled Students Allowance (DSA), which often requires a full diagnostic report as evidence for dyslexia and other specific learning difficulties.

The assessment is offered at a subsidised cost of £50 for eligible students – you may also be able to use your Inspire Fund to pay.
