Medical evidence

You need to submit medical evidence when registering with the Disability Inclusion Service and applying for funded support from Disabled Students’ Allowance (DSA). To claim DSA, your evidence must show:

  • That your condition/working diagnosis has lasted or is likely to last for at least 12 months
  • That your condition/working diagnosis has a substantial effect on your ability to carry out normal day-to-day activities, including education

We understand that getting evidence is not always a quick or simple process so we have different requirements for the evidence we accept. To receive support from the University, we just need evidence that indicates you experience a particular condition/conditions.

This allows you to gain timely support through our Disability Inclusion Service, even if you do not have DSA-acceptable evidence.

Evidence

Evidence accepted by both the University and by DSA

  • A DSA medical evidence form, filled out and signed by a medical professional
  • A University of Salford medical evidence form, filled out and signed by a medical professional
  • A signed/stamped doctor’s letter
  • A diagnostic report (such as for ADHD or Autistic Spectrum Conditions)
  • A referral letter for an ADHD or Autistic Spectrum Condition (ASC) assessment
  • An Educational Psychologist Assessment* for diagnoses of Specific Learning Difficulties such as dyslexia

Please note, all evidence submitted to DSA, no matter what type, must show:

  1. That your condition/working diagnosis has lasted or is likely to last for at least 12 months
  2. That your condition/working diagnosis has a substantial effect on your ability to carry out normal day-to-day activities, including education 

Medical evidence for the University of Salford does not necessarily need to show the above details.

*Please note: to claim support for a Specific Learning Difficulty, you must submit a full copy of an Educational Psychologist assessment or a report completed by a full report by a Specialist Teacher who is AMBDA qualified. If you suspect you may have a Specific Learning Difficulty but have not had either of the above assessments, you can receive one through the University.

 

Evidence accepted by the University of Salford (but not DSA)

  • A photo of your prescription/prescribed medication showing your full name and the date prescribed (as long as this medication is indicative of a particular condition/conditions) 
  • A copy of your medical history (as long as this is indicative of a particular condition/conditions) 
  • A sick note from a GP (as long as this is in-date and indicative of a particular condition/conditions) 
  • A hospital discharge letter (as long as this is indicative of a particular condition/conditions)
  • Medical evidence provided by your Wellbeing Adviser or counsellor.
