Accident, Near Miss, Hazard and Fault Reporting

All accidents, near misses and hazards must be reported to Health, Safety & Wellbeing without delay and always within 24 hrs, using one of the methods below.

  • Complete an Accident form online via the link below.Alternatively, if you do not have access to a computer - submit an
  • Accident Form to Health, Safety & Wellbeing (forms are available from the Health & Safety Co-ordinator).

If an accident has resulted in serious injury, or a hazard is so serious that it could result in an accident in the immediate future, it is important that these facts are reported immediately by contacting a member of the Safety Team directly.

Definitions for the purpose of reporting

  • An accident - is an unexpected event, which results in personal harm
  • Near miss - is a narrowly avoided accident
  • Hazard - is a dangerous situation or object, which has the potential to cause harm
