12-15 million people in the UK have a long term condition (LTC), which can range from musculo-skeletal problems, to degenerative neurological conditions and mental health issues. With an ageing population and an ageing work force, we will see an increased prevalence of chronic disease and long-term health conditions needing support and management by employers.

Comorbidities is one of the most important issues facing the health care system and employers. The number of people with multiple long term conditions is predicted to rise by a third by 2022.
The most prevalent long term conditions emerging as issues to manage in the workplace include:
- Mental health conditions
- Musculoskeletal conditions including arthritis
- Post cancer survivorship
- Cardiovascular issues
- Respiratory issues
Case management is 'a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet an individual’s health, care, educational and employment needs, using communication and available resources to promote quality cost effective outcomes’ (CMSUK Standards of Practice 2005).
A stepped and stratified case management model can be used to help employees living with a health condition, disability or after an injury, to stay in work and achieve optimal function within the workplace, while managing health issues. Early case management intervention is a valuable approach for businesses and healthcare providers a like.
Early intervention and best practice case management are key determinants in maximizing a return to work.
Gary Johnston currently works as a Senior Operational Advisor on Social Finance Projects, having gained over 25 years’ experience in both Australia and the UK in assisting employers and government develop evidence based return to work rehabilitation services that have substantial impact.
Over the last 10 years in the UK, Gary has won and operationalized large scale new services, including a catastrophic case management service for insurers, Individual Placement and Support Services, a UK wide absence management service for large employers, embedding a vocational rehabilitation service for cancer patients at the Christie Hospital and the development of a followership construct of leadership for an NHS Trust.
Gary has lead on projects to gain Care Quality Commission (CQC) and ISO9001 quality management accreditation.
Furthermore, Gary has lead on the development and integration of other innovative in work support systems, such as a psychological tele-health system for early support of health and well-being at work, a remote mindfulness service and a condition management programme for Work Choice and corporate clients.
In 2016 Gary completed an MBA in Leadership and Change and as part of this undertook a key project to develop an integrated digital and occupational health proposition for employers.
Gary is a long standing Trustee for the Vocational Rehabilitation Association and in his spare time enjoys eating way too much and dancing.
