About this placement

What is organised for students on commencement of placement?

We are a multidisciplinary team based in the community, whose remit is to avoid hospital discharge. Our client group ages range from 18 plus, but predominantly over 65.

On commencement of placement we will introduce you to your mentor and the team, we will orientate you to the building.

We will discuss the nature of our work and highlight potential spoke placements whilst you are with us, we will provide you with contact names and numbers for your spoke placements.

What are the arrangements for mentors/assessors?

We have three nurse mentors and a physiotherapist mentor, one OT starts the mentorship course in June. We take physiotherapy, nurse, paramedic and trainee GP students

The students tend to work alongside the mentors shift patterns. They are either 8.30-5pm , 8-8pm , it may be that they will work weekends also.


Notes: We are able to offer the opportunity to shadow other disciplines within the team, OT , Physio, Pharmacy technicians, social workers etc. We can also offer exposure to Mental Health as we have an RMN in the team as well as close contacts with the IMC Mental Health team.

What patient care situations are available in this placement?

The patients are assessed in either their own home or in A/E , patients can be supported in their own homes or placed within a NHS funded bed , the team follow the patients journey

What nursing model is used for planning care?

As a multidisciplinary team we do not have one distinct model, the model that most fits the teams philosophy is Orem, as we are a re-enablement service

We have staff who have undertaken the developing clinical skills module

Residential rehab.
